📄 Required Documents for Student Enrollment

For Parents or Legal Guardians of Students New to Lexington City Schools

When completing the online enrollment process, please have the following documents ready to upload. If you're unable to provide any required documentation, contact the school that serves your address for assistance.

✅ 1. Parent/Legal Guardian Photo Identification

Accepted forms of ID include:
- Driver’s License or State Identification Card
- Work Authorization Card
- Permanent Resident Card
- Passport
- Visitor’s Visa

🏠 2. Two Proofs of Residency (One from Each Category Below) - Note that addresses on both items must match.

Source 1 (Proof of Ownership or Lease):**
- Deed
- Current lease/rental agreement
- Closing statement
- Settlement statement
- Property tax statement
Source 2 (Recent Utility Bill):
- Gas, water, or electric bill **dated within the last 30 days**
  - Must show **both service address and mailing address**
- Bank Statement showing current mailing address
- Vehicle Registration Card
Alternative Affidavits are available below if unable to obtain the appropriate residency information or if your family’s enrollment situation requires additional verification:
- Form A – Student not suspended or expelled (Required for Lexington Senior High School)
- Form B – Residency Affidavit (Parent)
- Form C – Residency Affidavit (Caregiver) (used with Form B)
- Form D – Group or Foster Home Enrollment
- Form E – Landlord Verification (English) (Spanish)
- Form F – Homeless or Migrant Status Affidavit

📄 3. Child’s Birth Certificate

Only parents or legal guardians may register a student
Legal guardians must provide court-ordered custody or guardianship documents
- Not accepted: Notarized statements or powers of attorney

💉 4. Child’s Immunization Records

Students must meet immunization requirements within 30 calendar days of starting school
Students will not be allowed to attend after this grace period if immunizations are incomplete
- If available, scan and upload the records during the enrollment application process
- If not available at this time, you may bring to the school or email to your registrar or data manager within the first 30 calendar days of starting school

🩺 5. Health Assessment (K–12)

Required for all students entering NC public schools for the first time
Must be submitted within 30 calendar days from the student’s first day of attendance
A recent well-check from your provider (within the past year) may be used
Ask your provider to complete the official NC Health Assessment form, then submit it to your school

🎓 6. High School Students Only

Must submit an official transcript from the previous school showing all credits earned
Safe Schools Form
- This form is required and will be provided by the school your child will attend

👨‍⚖️ 8. Legal Guardians Only

Must present court-ordered custody or guardianship documents
Not accepted: Notarized statements or powers of attorney
